Use the Headers tab to provide information to be included at the top of the reclassification form.
To fill in this form:
- If you want to change the logo, move the pointer over the existing logo and click Replace. Use the Browse button to locate and select the graphics file. Click Open, then click Replace.
- In the Document Header Title field, select the existing title and type your own.
- Select the Include regulatory information for reclassifying equipment check box if you want the following statement in the header of the reclassification form. "Under 29 CFR 1910.146 (c)(7), sites can temporarily reclassify permit spaces that have no actual or potential atmospheric hazards and where all non-atmospheric hazards can be eliminated from outside the space. This form is intended to guide and document permit-space reclassification."
