Confined Space - Headers Tab

Use the Headers tab to provide information to be included at the beginning of the confined space assessment and to associate a lockout procedure with the confined space.

To access the Headers tab see Confined Space - Create New Assessment or Edit.

Note: Only predefined text is translated. Custom content is not translated.

To fill in this form:

  1. In the Space ID # field, type an identification number for the assessment. Numbers, letters, and symbols are allowed. When filling out a new assessment, the Space ID # is automatically filled in with the Equipment ID of the equipment specified on the Description tab.
  2. In the Associated Equipment field enter equipment that resides in or is otherwise associated with the confined space. For example, if the confined space is a tank, then valves are likely associated with the space.
  3. Click Add Image to include a picture of the confined space. Click either Upload From Computer or Choose From Google Drive. If uploading from Google Drive and the selection window does not open, check your browser for blocked popups. Once the graphic is selected, click one of the following buttons:
    • Click Insert + Edit to edit the picture. You can adjust the brightness or contrast, crop or rotate the picture, and add labels.
    • Click Insert 'as is' to add the picture without making any edits.
  4. If desired, type a title in the text field above the image. If you want to edit or remove the image, click the grey down arrow in the lower right corner of the image and select Edit or Delete.
  5. In the Assessor Name(s) field enter the name of the person who assessed the confined space. That is, the person who provided the information for this form.
  6. Click the calendar icon next to the Assessment Date field to fill in the date that the confined space was assessed.
  7. If a lockout procedure is required to secure the space, select the lockout procedure from the Associated LO/TO Procedure list. Only published procedures for this equipment are included in the list. This includes procedures with a status of New Version in Progress which means that someone is editing the previously published procedure.
  8. If you want to include a note select Include Notes / Special Considerations. Select a pre-defined note from the list or begin typing in the box below to enter your own.
  9. Select Include Lockout Warning to include a warning about a required lockout procedure. You can accept the pre-defined text or add your own.
